These resources have been designed to support you while guiding youth through program materials, helping them develop healthy habits, and connecting them with resources that may improve their well-being.
It is important to remember (and communicate) that you are not a trained therapist and should not provide counseling to youth in place of a mental health professional. Rather, you are a coach who can help them consider their well-being needs and connect them with the professionals and resources that can help.

The conversation guides provide structured, youth-centered discussions to help you create personalized well-being plans and have regular check-ins.

The worksheets offer interactive and reflective activities for youth to explore and track their progress across different dimensions of well-being.
Guide to Holistic Well-Being
The Guide to Holistic Well-Being outlines the eight dimensions of holistic well-being, offering guidance, reflection prompts, and resources to help youth grow in each area.
If you have any questions or challenges that these resources do not address for you, ask them in the Coach Community Group. The Coach Community Group connects all participating Soul Care Coaches with one another and with the team at the Center who are overseeing the implementation of Soul Care. If you have not yet been added to the group, let your supervisor know.
Additionally, you are invited to attend the regular ‘Navigating Care Conversations’ sessions where you can hear how other coaches are approaching these conversations, ask questions to the team at the Center who are overseeing Soul Care implementation, and hear from organizations across the City who run programs that your youth may be interested in.
For most services in the Resource Hub, we ask that you support the youth in getting registered (if no registration is required, help the youth save the event in their phone and ensure they meet any requirements).
Resources with an asterisk are provided by the Center for Fair Futures as part of the Soul Care program and may have limited availability. Use the link below to send a request for that service.
If you are aware of publicly available programs that most youth will have access to, please add it to the Resource Hub for other coaches to refer their youth to.
In addition to using the Resource Hub, we recommend staying up-to-date with the programs your agency is running that may benefit the youth in your care as well as looking online and in your neighborhood.
